As mid level providers in Tennessee, Physician Assistants and Nurse Practitioners can evaluate, diagnose, treat and prescribe for patients. We are skilled in primary care and are dedicated to providing comprehensive, compassionate and excellent medical care for our fellow Memphians. Telehealth, Chronic Care Management and Remote Patient Monitoring available. We offer televideo appointments, management of chronic diseases and remote patient monitoring. The remote monitoring records your readings via bluetooth to the web and allow us to see your numbers real time. This enhances the telemedicine visit and allows us to make changes to better address your health needs. If you are interested, please contact the front desk and we will either have the monitors shipped to you or you can pick up in clinic. Under our chronic care management program, we coordinate with specialists and are available by phone outside your appointment time to tend to your needs.
